Cheri & I are involved in the collation and shipping of bloods to Cornell University to testing.

The forms below are what you need to complete to enable you to send your dogs bloods over, there is also a FAQ that should answer most of your questions, however if you have questions that are not answered, please email us.

You need to complete each of the "Forms" below, you also need a letter from your vet confirming that the bloods are not contaminated (see FAQ).
